1-800-Call-DHL (255-5345) and request that shipping labels be delivered to your office.

To ensure that shipping costs are charged to Harmony Dental, please follow these instructions when completing your airbill:

• Under "method of payment", check the "bill receiver" box. Call us for our DHL account number and enter it on the line marked "DHL receiver account number". This will ensure that the case is billed to Harmony Dental.
• In the "declared weight" line, enter "1 pound".
• In the "shipment valuation", enter "N/A".
